Berkeley Lab uses the Representations and Certifications form to accurately capture the profiles of organizations interested in or already pursuing research projects with the Lab. The information on the form is crucial for determining foreign affiliation and fulfilling DOE contractual requirements.
Who must complete the form?
Not every research partner is required to complete the Representations and Certifications form. The following partners or sponsors are exempt from completing this form:
- Integrated Contractors (DOE National Laboratories)
- U.S. Federal Government
- DOE (Dept. of Energy)
- State of California
- State (states other than California) or Local Governments
Validity
Each fully completed and signed Representations and Certifications form from a qualified sponsor is valid for two years from the date of signature. Consequently, these documents must be recertified every two years.
New Sponsor Requests & Existing Sponsors Linked to New Proposals and Proposal Modifications
For every new sponsor request, new proposal, or proposal modification in eSRA involving a direct sponsor with missing or outdated Representations and Certifications, the Sponsor Administrator at the Strategic Partnerships Office (SPO) will send a four-page Representations and Certifications form via ShareFile (for secure handling) to the designated point of contact. This request is typically sent within one business day of the initial notice.
ShareFile will provide automated weekly reminders to ensure the form is completed and returned by the due date. Please be aware that the initial request expires after 30 days. The Sponsor Administrator monitors these deadlines and will alert the requesting Resource Analyst (RA) of any upcoming expirations to determine if a renewal is required.
